1. General Use

  • All services provided by Clear Stream Health are intended for personal wellness and are not a substitute for medical diagnosis or treatment.

  • By booking or purchasing any of our services, you confirm that you are at least 18 years of age or have the consent of a legal guardian.

  • Clear Stream Health reserves the right to refuse service to anyone for any reason permitted by law.

2. Appointments & Payments

  • All appointments must be booked in advance through our website, by phone, or in person.

  • Payments must be completed at the time of booking unless otherwise agreed.

  • Late arrivals may result in shortened sessions without refund.

3. Memberships

Clear Stream Health offers exclusive membership plans for recurring wellness treatments. By enrolling in a membership, you agree to the following terms:

  1. Commitment Period

    • Memberships require a minimum commitment of 6 consecutive months before cancellation is permitted.

    • Early cancellation requests within the first 6 months will not be accepted.

  2. Billing & Payments

    • Memberships are billed monthly to the payment method on file.

    • Missed or failed payments may result in suspension or termination of membership benefits.

  3. Benefits & Usage

    • Membership benefits (infusions, vitamin injections, discounts, etc.) are valid only during the active membership period.

    • Unused benefits do not roll over to the following month unless explicitly stated in writing.

    • Discounts included in the membership apply only to the member and are non-transferable.

  4. Cancellation After 6 Months

    • After the 6-month commitment, members may cancel at any time with at least 30 days’ written notice.

    • Cancellation requests must be submitted by email to info@csh2024.com or in person at our office.

  5. Upgrades & Downgrades

    • Members may upgrade their membership at any time.

    • Downgrades are allowed only after the 6-month commitment period.

  6. Freezes & Pauses

    • Memberships may be temporarily frozen for up to 1 month per year for travel or medical reasons, with prior written request and approval.

    • During a freeze, billing is paused and membership benefits are not accessible.

4. Refunds & Cancellations

  • Service fees and memberships are non-refundable except as required by law.

  • Refund requests for exceptional cases will be reviewed individually.

5. Health & Safety

  • Clients must disclose any medical conditions, allergies, or medications prior to treatment.

  • Clear Stream Health is not responsible for adverse effects resulting from undisclosed medical conditions.

6. Liability Disclaimer

  • By using our services, you acknowledge that treatments are elective and carry potential risks.

  • Clear Stream Health, its staff, and affiliates are not liable for any direct or indirect damages resulting from services provided.
